A DOT physical (Department of Transportation physical) is a federally required medical exam designed to ensure that commercial drivers are physically and mentally fit to operate large or heavy vehicles safely on public roads. In New Jersey, this exam follows FMCSA (Federal Motor Carrier Safety Administration) standards and is required for anyone holding—or applying for—a Commercial Driver’s License (CDL).

Why the DOT exam is important

The goal isn’t just paperwork—it’s public safety. Commercial vehicles are large, heavy, and often travel long hours, so even minor medical issues (like uncontrolled blood pressure, vision problems, or sleep disorders) can significantly increase the risk of accidents. Who actually needs a DOT physical in NJ?

You need a DOT medical exam if you:

  • Hold or are applying for a CDL (Class A, B, or C)

  • Drive a vehicle weighing over 10,001 lbs for commercial purposes

  • Transport hazardous materials requiring placards

  • Carry 9+ passengers for pay (like a bus or shuttle driver)

  • Operate interstate commercial vehicles (crossing state lines)

  • Drive certain regulated intrastate commercial vehicles in New Jersey

Even some local drivers who never leave NJ still need it if their job falls under FMCSA or NJ MVC commercial regulations.
