DOT Physical Exams

Special - $90 DOT Exam Fee

A DOT physical exam is required by the FMCSA for anyone operating a commercial motor vehicle.

DOT Exams Include:

Step 1: Fill out a brief health history prior to your DOT exam appointment.

Step 2: Complete your physical exam with Dr. Joseph Kuna, D.C. who is a certified medical examiner for the Federal Motor Carrier Safety Administration (FMCSA).

  • Review of medical history, any special medical conditions, and all medications.

  • Height, weight, blood pressure, pulse rate, breathing, range of motion (ROM), vision, and hearing exam.

  • Urinalysis

Step 3: Paperwork and documentation of all exam results. You will receive two copies of the results - one for yourself, and one for your company.

What you will need:

  • Photo ID

  • Medical records

  • List of Medications (prescription and OTC), including strength and dosage.

  • Complete contact information for all your doctors

  • Glasses/Hearing Aids

If you have been diagnosed with a heart condition or sleep apnea, we will also need the following:

  • Copy of your most recent stress test (heart conditions)

  • Copy of most recent sleep test results (sleep apnea)

Spinal Manipulation

Soft Tissue Therapy

Massage to relieve tension within muscles causes by injury or chronic misuse of joints. Used on muscles, tendons, and ligaments decreasing stiffness and increasing joint mobility.

Theragun

Handheld percussive massage device used for deep muscle massage. Releases tension and knots, relieves pain and discomfort, increases mobility and range of motion.

Sombra Warm Therapy

Natural pain relieving topical gel used for temporary relief of minor aches and pains of muscles and joints associated with back pain and arthritis.

ArthroStim®

Chiropractic adjustment instrument used to provide gentle low-force pressure and “thrusts” to the affected areas requiring adjustment.

Flexion Distraction

A gentle safe treatment for spinal pain relief by reducing intradiscal pressures and widens the spinal canal while returning motion to the spinal joints.
